Department of Oncology, 2nd Medical Faculty, Charles University and University Hospital Motol

The Oncology Clinic of the 2nd Faculty of Medicine of the UK and the Motole National Academy of Sciences is one of the Comprehensive Oncology Centers in the Czech Republic, performing comprehensive multidisciplinary treatment of cancer diseases (solid tumors and lymphomas) in cooperation with other specialist workplaces at the FNM, ÚVN, NNH and the Proton Center. It is one of two centers in the Czech Republic focusing on the treatment of rare tumors and is a member of the European reference network for the treatment of rare tumors. The Oncology Clinic is a teaching workplace for the 2nd Faculty of Medicine of the Charles University and the Faculty of Nuclear and Physical Engineering of the Czech Technical University. It cooperates with the International Atomic Energy Agency on the training of foreign doctors and physicists in the field of radiotherapy. At the clinic, research into new medical preparations is underway, patients are being included in an international clinical trial.

The institution is a member of the Czech Cooperative Lymphoma Group (CLSG), the European Organization for Research and Treatment of Cancer (EORTC) - a working group for the treatment of sarcomas. The clinic cooperates with patient organizations Lymphoma HelpMom Help, ČAVO (Czech Association for Rare Diseases).

The outpatient clinics of the Oncology Clinic of the 2nd Faculty of Medicine, Charles University and the Motol University Hospital are located in the main building of the FNM on the 3rd floor at node D.

Every new patient who is treated at our clinic goes through this part of the clinic.

At the first visit, he is first taken in by the nurses at the reception, where they write down his basic data with the patient and check with which health insurance company he is registered. He is then admitted by a doctor who takes him and treats him.

There is an initial examination and the planning of additional examinations. Each patient is assigned a treating physician.

At the initial examination, the patient is acquainted in detail with the type of disease and its extent. At the same time, treatment is proposed. If the patient agrees to treatment, treatment is initiated.

Each patient signs an informed consent stating how information about his illness can be handled and which of his relatives and friends can be informed about his illness.

At the outpatient clinic, blood samples are taken for blood tests, biochemical tests and urine tests.

These examinations are part of both the controls during the treatment of the disease and after its termination and the long-term monitoring of the patient.

Long-term patient monitoring - dispensarization includes regular check-ups at various intervals in accordance with standard procedures. When the patient's condition worsens, checks are performed at shorter intervals as required by the patient's condition.

At the outpatient clinic, the cancer is treated in the following ways:

The bed part of the Oncology Clinic of the 2nd Faculty of Medicine of the University of Warsaw and the Motol General Hospital consists of 2 stations with a total of 30 beds.

Bed stations have 30 beds - 9 triple rooms, 1 superior single room and 1 superior double room

The rooms are equipped with central oxygen distribution, most are equipped with TV, accessories - toilet, shower and bathroom sinks are common for 2 adjoining rooms. There is a TV room at each station, and there is a shared fridge at each station in the hallway. A stay in a superior room, which is equipped with bathroom and accessories, refrigerator, plasma TV and telephone, is charged.

Trained medical staff work at both stations; 3 doctors work at each station. Furthermore, a physiotherapist, social worker, diet nurse, psychologist and clergyman regularly attend each station. Dilution and preparation of cytostatics is provided centrally in the hospital's pharmacy. During the stay in the inpatient department, in addition to standard diets, it is possible to provide above-standard meals, of course with regard to possible dietary restrictions; it is also possible to arrange above-standard visits by a pedicurist, manicurist, hairdresser, clergyman and seller of the daily press.

Hospital

It is a separate part of the ambulatory operation, where at regular time intervals the patient undergoes planned treatment, most often chemotherapy. It is located on the 3rd floor, communication node B.

The hospital is equipped with 20 beds and 7 application chairs, on which the treatment takes place.

Chemotherapy is most often given by intravenous infusion.

The patient is in the care of a nurse who prepares the medication and applies it under the supervision of a doctor.

The duration of application varies as required by the treatment protocol, not longer than 6 hours. After the application, the patient goes home and his condition does not require hospitalization.

Scope of activity

At the workplace, we perform all techniques of intracavitary, interstitial, intraluminal and superficial brachytherapy, especially for tumors of the following locations:

  • cervix
  • uterine body
  • vagina
  • vulva
  • prs
  • bronchus
  • ret
  • skin
  • nasopharynx
  • esophagus
  • anal canal
  • lower part of the rectum
  • bile ducts
  • penis
  • soft tissue sarcomas
  • and preparations for the initiation of HDR prostate brachytherapy are completed

Statistical data

We perform 500 - 600 brachytherapy applications a year at the workplace. A significant part is the application of intracavitary brachytherapy in gynecological tumors, where brachytherapy forms a substantial part of radiation treatment protocols. As the number of breast-sparing operations increases, so does the number of interstitial breast applications. Indications for irradiation techniques are very strictly individually indicated.

Organization of work

Invasive techniques (interstitial brachytherapy, intracavitary uterine techniques) are performed under general anesthesia, which is provided by the anesthesiology and resuscitation clinic.

We perform application techniques requiring endoscopy in close cooperation with the relevant specialists.

The department has developed a quality assurance program both in terms of physical and technical, as well as in terms of organization and clinical. The responsibilities of individual employees for partial process steps are determined in detail and standard procedures for individual application techniques are developed. The irradiation plan is made for each application individually. A detailed database of individual patients and performed procedures is maintained to evaluate the results of treatment.

Ordering patients for brachytherapy treatment

Patients are indicated for irradiation of brachytherapy according to treatment protocols, either for separate brachytherapy or as part of combined radiation therapy with external radiotherapy. Brachytherapy is classified based on clinical need and there are virtually no waiting times. The order time is governed only by the connection to external radiotherapy or the healing time for postoperative irradiation. In addition to patients from our catchment area, we also irradiate patients from more distant areas with brachytherapy, where special brachytherapy techniques are not available.

One of the basic components of oncological treatment is radiation - radiotherapy. The section of external radiotherapy - telotherapy (irradiation room) is located outside the main building of the Motol University Hospital - on the ground floor of the building of the new pediatric oncology (see map, building no. 23). This section is divided into a section where preparation for irradiation takes place (simulator, planning computer tomograph / CT /, planning section, model laboratory) and own irradiation rooms with ambulances. The department is equipped with three irradiators - linear accelerators, which use photon radiation X 6 and 18 MV (Linac 1, 2) or 4 MV (Linac 3) for radiation, or radiation with accelerated electrons with energies 6,9,12,16 and 20 MeV (Linac 1, 2).

We irradiate about 1200 new patients a year. We provide both radical and palliative treatment, irradiation can take place as a treatment alone or in combination with brachytherapy or chemotherapy, it can be classified as either preoperative or postoperative treatment. The basic irradiation technique is 3D conformal radiotherapy, ie. radiation therapy, based on three-dimensional planning, where the irradiated volume is as close as possible to the treated area. Therefore, multiple input fields are used, which are shaped, and the dose distribution in the target volume and surrounding organs is evaluated according to dose volume histograms.

At our clinic, we also irradiate pediatric patients, and we also perform whole-body irradiation of children in preparation for a bone marrow transplant. Another special technique is stereotactic fractionated brain radiotherapy. We perform radiation intensity modulated radiotherapy (IMRT) and image-guided radiotherapy (IGRT), in which we use an additional kilovoltage device attached to a linear accelerator.

Patients are sent for irradiation based on the decisions of consiliar examinations or multidisciplinary oncology teams. To decide on the method of radiation treatment, it is necessary to know complete information about the patient (anamnesis, concurrent diseases, the course of the current disease, imaging results, surgical findings, histological examination, imaging documentation) at the first visit of the patient to the irradiation facilities. Based on the examination of the patient and his documentation, the patient is focused on irradiation, in some cases including the production of special fixation aids, and then a planning CT examination. The patient is informed about the method of radiation treatment, receives instructions about treatment, and signs consent to radiation treatment.

At the second visit, the prepared irradiation plan is checked on the patient - the simulation of the irradiation plan on the radiotherapy simulator. Irradiation parameters and images of irradiation fields are entered into a computer verification system and are used for inspection at each irradiation.

The actual irradiation usually takes place daily except Saturdays and Sundays for several weeks.

The patient is regularly checked by a doctor, and blood tests are performed as needed. Checking the correct position of the patient during the irradiation cycle is performed by repeated imaging on the irradiator and by comparing the acquired images with reference images.

Hardware

Three Truebeam linear accelerators (Varian) enabling the implementation of irradiation techniques with modulated beam intensity - IMRT (Intensity Modulated Radiotherapy) and VMAT (Volumetric Modulated Arc Therapy). To check the position of patients, the accelerators are equipped with additional imaging systems - megavoltage (MV) imaging system and kilovoltage (kV) imaging system, allowing to perform cone-beam CT (Cone Beam Computed Tomography). The accelerators are equipped with a system for radiotherapy controlled by the patient's breathing movements.

The two Truebeam accelerators have 6MV and 18MV photon beams, 6MV and 10 MV flattening filter free (FFF) photon beams and 6 to 20 MeV electron beams. They are equipped with a Millenium 120 Multileaf Collimator (MLC). Both accelerators were put into operation in 2019.

The third Truebeam accelerator produces 6MV photon beams and is equipped with a HD MLC - High Definition Multileaf Collimator with a smaller blade width. This accelerator was put into operation in 2020.

Procedure for ordering new patients to the Oncology Clinic of the 2nd Faculty of Medicine of the UK and Motol Medical Center

This procedure applies to the following situations:

  • A doctor or patient (or his family) requests to take over an oncology patient under the care of the Oncology Clinic of the 2nd Faculty of Medicine of the UK and the Motol General Hospital

  • The patient requests a second opinion on current and/or planned anti-tumor treatment (second opinion).

In order to order a patient, we require sending or delivering information about the reason for the order, the treatment to date and the examinations performed. In addition, it is necessary to provide contact information for the referring doctor (e-mail address and/or telephone number) and contact information for the patient (always telephone number or e-mail address).

The documentation should contain an overall report on the current course of the disease and its treatment, histology, possibly the surgical protocol, the results of essential examinations (laboratory examinations, descriptions of CT or other imaging examinations, etc.)

  • By mail with documents attached as scans or photos. The email address of the clinic is onkologie@fnmotol.cz
    In person - bring to the oncology clinic file (main building of FN Motol, node D, 3rd floor)

  • By mail to the clinic address

  • In exceptional cases, the clinic can be contacted at 224434760

After sending the patient's documentation, the patient and/or the referring doctor will be informed within 5 working days whether the patient can be taken into our care and the date (date, time) of the patient's first visit to the oncology clinic.

Please note that the capacities of our clinic are limited and when deciding whether to accept a patient for treatment, we also take into account the availability of adequate oncology care closer to the patient's place of residence.

  • Ambulance are located in the main building of the National Property Fund on the 3rd floor on node D.
  • Inpatient stations they are located in the main FNM building on the 3rd floor at junction B and C
  • Brachytherapy is located in the main building of the National Property Fund on the minus 3rd floor at node A
  • Hospital is located in the main building of the FNM, on the 3rd floor, communication node B

In all these cases, it is an exit station Motol Hospital.
