Department of Pulmonology, 2nd Faculty of Medicine, Charles University and University Hospital Motol is a facility in the field of pneumology and phthisiology, which deals with prevention, diagnosis, treatment, assessment and research of all respiratory diseases, including tuberculosis.
The field also focuses on reducing the transition from the acute phase of the disease to chronic and rationalizing the care of people with chronic respiratory diseases.
The clinic teaches pneumology for students of the 2nd Medical Faculty of Charles University III., IV. and VI. year, including foreign students, led by teaching staff, and all second-certified physicians are involved.
The clinic is involved in the system of postgraduate education, its employees have lectures at the IPVZ, bronchology internships for beginners and advanced, cytological internships and intensive care internships take place at the clinic.
The staff of the clinic actively participates in professional meetings at home and abroad and is active in publishing.

The outpatient unit includes an acute outpatient clinic for suddenly aggravated patients with lung disease, when the outpatient pneumologist has exhausted all possibilities for its treatment, this outpatient clinic is also an admission outpatient clinic, so please understand and respect the rule that patients are determined by a doctor and patients entering hospitalization. For these reasons, we ask patients and colleagues to try to solve as many cases as possible in specialized clinics in the place of residence.
Furthermore, a bronchological department, a functional laboratory and a cytology laboratory are registered in the outpatient department.

The clinic for adult patients with cystic fibrosis at our workplace started its activities in December 1987 and until 2000 it was the only pneumological workplace of this kind in the Czech Republic. The number of adult patients with cystic fibrosis has increased in recent years, so we now have more than 100 of these patients in care. Our biggest problem since 1994 is the presence of the epidemic strain ST32 Burkholderia cenocepacia. The proportion of patients colonized by this strain reached 1999% in 53. Since then, thanks to the introduction of a comprehensive hygienic-epidemiological regime, it has been gradually reduced, currently standing at 7%.
We take care of patients from the children's part of the CF FN Motol Center after reaching adulthood, as well as patients diagnosed after the age of 18. Diagnosis consists of careful evaluation of the clinical picture, determination of chlorides in sweat and genetic examination. All modern procedures, both pharmacological and non-pharmacological, including lung transplantation, are used in treatment. Cooperation with other departments of the Motol University Hospital is of course necessary, for all of them we should name the Department of Pediatrics, Department of Biology and Medical Genetics, Department of Rehabilitation and Sports Medicine, Department of Medical Microbiology and III. surgical clinic.

Diagnosis and treatment of bronchogenic carcinoma is one of the main programs of the clinic, it is performed on patients from the catchment area of the clinic and throughout the Czech Republic and their number is constantly growing. Patients who are diagnosed with the disease at an early stage are in close cooperation between Pneumologická and III. surgical clinic FNM sent for surgical solution. Patients with already locally advanced disease are treated with chemotherapy and radiotherapy in cooperation with the radiotherapy department, treatment is indicated with regard to their quality of life. In the treatment we also apply the possibilities of interventional bronchology (laser treatment, airway stenting) and in cooperation with the radiotherapy department brachytherapy.
Since 2007, we have been one of the nine pneumooncology centers in the Czech Republic that can provide treatment linked to centers (Alimta, Avastin, Iressa, Tarceva).
Our clinic is also conducting international studies, thanks to which we can provide suitable patients with the latest, as yet unregistered treatment.
One doctor with a certificate in clinical oncology works at the clinic, and most patients are treated on an outpatient basis.

Patients with interstitial lung processes are monitored and treated for a long time in a specialized outpatient clinic. These diseases represent a heterogeneous group of chronic non-tumor diseases, affecting the area of respiratory bronchioles, alveolar ducts, alveoli, pulmonary capillaries and pulmonary interstitium. So far, more than 160 nosological units have been described in the literature. The basic series is idiopathic interstitial pneumonia, sarcoidosis, pulmonary interstitial involvement in systemic connective tissue disease, pneumoconiosis, drug-induced lung involvement and exogenous allergic alveolitis.
Our outpatient clinic offers, in cooperation with other branches of the Motol University Hospital, comprehensive diagnostics and treatment of these rare diseases.

The doctors of our clinic are members of the transplant team, whose other key part is experts III. surgical clinics of the Motol University Hospital, with which there is close cooperation. At our workplace, patients who are considered as candidates for lung transplantation are examined. Patients with end-stage lung diseases such as COPD, pulmonary fibrosis, cystic fibrosis, lymphangioleiomyomatosis, patients with primary pulmonary hypertension and other diagnoses are indicated for this treatment. Patients are sent for a comprehensive examination from various workplaces in the Czech Republic. Examination of the patient and preparation for inclusion on the waiting list requires multidisciplinary cooperation. We work with a psychologist erudite in transplant medicine, a nephrologist, an infectionist, a cardiologist, an endocrinologist and other specialists. In individual cases, the help of a dietitian is required, who manages the patient's reduction or realization regime. We consult complicated patients with thoracic surgeons from the transplant team already during the examination program. Definitely at the end of the examination process, the entire transplant team decides on the suitability of including the patient on the waiting list for lung transplantation. After the transplant itself, we participate in post-transplant care of patients.

Since 2005, the Center for Tobacco Addiction Treatment has started operating at the Department of Pulmonology, University Hospital Motol, as one of the first five in the Czech Republic. The head of the program is a physician who has undergone training at the IPVZ and also works closely with the VFN Reference Center and continuously adds knowledge and expertise. The goal of the Center is the treatment of tobacco dependence and effective help in quitting smoking with the use of nicotine or Champix replacement therapy. The center treats not only patients at the Motol University Hospital, but also patients who seek our help on the recommendation of their general practitioner or find information about the centre's activities on the Internet. The center also deals with the treatment of tobacco dependence in specific groups - school children and youth, pregnant women. The Centre's office hours are every Wednesday from 14 pm and every Friday from 13 pm in the clinic's outpatient clinic. Clients do not have to have a recommendation from a general practitioner or another specialist, an identity card and an insurance card are enough. Introductory lessons, when the degree of nicotine dependence is determined, initial examinations are performed, the client is acquainted with the withdrawal methodology and suitable medication is determined, it takes about 30-45 minutes, control examinations are then in the range of about 10 minutes, about 4 times in 3 months.

The specialized outpatient clinic of the lung clinic is an outpatient clinic for difficult-to-treat asthma
(OLA), which is one of the workplaces of the National Center for Severe Asthma (NCTA).
This center brings together experts in the field of allergology and immunology, Eva Vernerová, MD, and pneumologists. Alena Vlachová, MD, prescribes for adults in the premises of the pulmonary clinic; for the pediatric population, prof. MUDr. Petr Pohunek, CSc.
It is possible to send to us for examination of a patient whose asthma cannot be brought under therapeutic control by standard therapy. Depending on the nature of asthma, a specific therapy of the patient will be selected.
In addition to specific immunotherapy, it is possible to indicate a patient from this workplace to perform bronchial thermoplastics, ie. treatment of bronchial smooth muscle enough to improve the patient's quality of life and help control his illness.
The center uses all clinics and other FN workplaces to help with diagnosis and treatment.

The bronchological workplace is one of the most important in the Czech Republic in terms of output and equipment. Most bronchoscopies are performed under local anesthesia, for more demanding procedures general anesthesia under jet ventilation is used. In addition to standard fibrobronchoscopy with biopsy, diagnostic procedures also include bronchoalveolar lavage, mediastinal node puncture and transbronchial lung biopsy. In November 2008, the workplace was equipped with the EBUS system for mediastinal node puncture under direct ultrasound control. Biopsy material is examined on site by the ROSE method (rapid on-site cytological evaluation). The combination of these methods speeds up and refines the diagnosis of bronchogenic carcinoma and other diseases and often saves the patient more demanding surgical diagnostic procedures.
The bronchology department provides urgent procedures for other departments of the hospital, such as aspiration of secretions from the airways of patients in intensive care units, assistance in surgical rooms for certain types of procedures and assistance to anesthesiologists in difficult intubation. This service is available 24 hours a day. In the field of interventional bronchoscopy, dilatations of the trachea and bronchi and desobliteration procedures are performed using a laser or electrocautery. Silicone, wire or combined stents are being introduced. Endobronchial brachyradiotherapies are applied in cooperation with the radiotherapy department. We cooperate with surgical clinics in the treatment of postintubation tracheal stenoses. Three bronchoscopic doctors are erudite in all types of bronchoscopic procedures, including interventional ones, another 5 doctors perform only fibrobronchoscopy. There are 3 endoscopic nurses working in the department.
On November 17, 11, the department obtained the accreditation of ČLK for training in medical performance and for issuing functional licenses for the field F 2004 - rigid and interventional bronchoscopy.

Since 2003, a sleep laboratory has been operating at the pulmonology clinic, examining patients with suspected obstructive sleep apnea syndrome. This disease is characterized as a sleep-disordered breathing disorder, in which respiratory arrest occurs due to the collapse of the upper respiratory tract. During such an apnea pause, there may be a decrease in blood oxygen saturation and other pathological phenomena. Stopping the breath is followed by rapid deep breathing, sometimes waking up. The arrests are repeated during the night, sleep is poor, the patient snores loudly and suffers from excessive daytime sleepiness.
Examinations in our sleep laboratory are preceded by short ones outpatient visit, during which the patient 's problems are discussed with the doctor, the patient is also examined for spirometry, and undergoes blood pressure, pulse and blood saturation measurements with a pulse oximeter. To order (224436646) we require a recommendation from a general practitioner or specialist (pulmonologist, cardiologist, ENT doctor…)
The examination itself then takes place after one-day hospitalization, starts at 22 pm and ends at 6 am the next day. The patient is monitored in a single room, bathroom is available in front of the room. The laboratory is equipped with a Porti 7 device, the examination is non-invasive. During overnight sleep monitoring, the patient wears a chest and abdomen belt, as well as heart rate, body position and the presence of snoring. On the finger there is a sensor measuring the oxygenation of the blood, on the nose a tube monitoring the air flow during breathing. The sleep laboratory has a capacity of 6-8 examinations per month.
Our laboratory deals exclusively with diagnosis and non-surgical therapy of obstructive sleep apnea syndrome using pressure therapy (device with nasal or full face mask CPAP, BiPAP). Other sleep disorders - such as insomnia (insomnia), narcolepsy (somnolence), restless legs syndrome, etc. are not the subject of our activities.

The cytology laboratory falls under Department of Pathology and Molecular Medicine, Charles University, 2nd Faculty of Medicine and Motol University Hospital, processes material collected during bronchoscopies, pleural punctures and other biopsies. It is equipped with 3 microscopes, 2 cytologists with a certificate for clinical cytology work here.

The functional laboratory performs spirometric examinations, body plethysmography, bronchodilation and bronchoconstriction tests, diffusion examinations. At the same time, a 6-MWT (six-minute walk test) is performed here, which is also newly used to indicate home oxygen therapy with liquid oxygen for mobile patients. We also perform FeNO examinations at the workplace. All of the above tests are used to diagnose and monitor lung function in diseases associated with ventilation disorders. Preoperative assessment of lung function in patients who are to undergo thoracic or peritoneal surgery is irreplaceable. We are increasingly performing functional examinations of patients after lung transplantation and after volume reduction operations.

The inpatient part of the Pneumology Clinic consists of 3 standard wards and an intensive care unit with a total capacity of 70 beds (3x20 beds and 10 ICU beds).
Intensive Care Unit
The intensive care unit was opened at our workplace in February 1993. It provides care to patients with a full range of acute and life-threatening conditions in pneumology. These are mainly exacerbations of chronic obstructive pulmonary disease and bronchial asthma, pneumonia, fluidothoraxes, including thoracic empyema, pneumothoraxes, pulmonary fibrosis, hemoptysis and large airway stenosis, including observation of patients after interventional bronchological procedures.
At present, the ICU has 10 beds, three Rafael Silver fans (invasive and non-invasive ventilation) and two Philips Respironics V60 (non-invasive ventilation) devices are available for mechanical ventilation. In recent years, about 300 patients are hospitalized in the ICU per year, mechanical ventilation is indicated in about 50 of them.
Cooperation with other departments of the Motol University Hospital mainly concerns the Department of Anesthesia and Resuscitation, the Department of Imaging Methods (Interventional Radiological Procedures), III. surgical clinics and internal medicine clinics.
Standard department
The rooms in the standard wards have a maximum of 3 beds and have their own facilities, which they share with the adjoining room, with the exception of 3 rooms only with their own bathroom, which can be used to isolate the sick. The rooms are equipped with a TV set. In the larger ward, there is one single superior room, which is equipped with a telephone, computer socket, refrigerator and television. The department has an exercise bike, which is available to patients for rehabilitation, possibly with the assistance of a rehabilitation worker who comes to the department. Walking patients can use the dining room with TV and the library.
The departments are equipped for the treatment of non-tuberculous lung diseases (pneumonia, cancer, congenital and acquired lung disorders, circulatory diseases). Since June 2004, we have only been diagnosing patients with suspected tuberculosis at the clinic and then transferring them to a specialized medical institution for follow-up care. As part of the care, a social worker, a dietary counselor and spiritual comfort to the clergy are available to the patients according to the patient's wishes.
The pulmonary ward was opened in the Motol hospital when it was established on January 4.1.1943, 1943. The first primary was the renowned Czech pneumophthisologist prof. MUDr. Jaroslav Jedlicka, DrSc. (1945-1945). Other managers were Prim. MUDr. Antonín Platil (1978-1978), doc. MUDr. Jan Münz, CSc. (1984-1984) and MUDr. Vladimir Mikulenka CSc. (1992-XNUMX).
At first, it was the largest ward of the Motol hospital with 230 beds, but gradually the number of beds decreased with the decrease in the number of patients with tuberculosis and the opening of other lung wards in Prague and its surroundings. At the same time, however, there was an increase in the number of non-tuberculosis patients. Since 1979, the counseling center for patients with bronchial asthma and chronic obstructive pulmonary disease has been operating, and since 1987, the counseling center for adult patients with cystic fibrosis has been the first in the Czech lands. Bronchoscopies have been performed since the late 50s, and the workplace has had its own functional laboratory since 1981. In 1990, the inpatient department was moved from the already unsuitable barracks hospital to temporary premises in the building of the Hospital for the Long-Term Ill. In the same year, pre- and postgraduate teaching takes place at the department.
The new TRN pavilion (Department of Tuberculosis and Respiratory Diseases) was opened in 1992 and, thanks to Prim. Mikulenka, it was one of the most modern pneumological workplaces in the Czech Republic. However, Mikulenka did not see the opening of the intensive care unit in 1993. Other managers were doc. MUDr. Vladimir Vondra, DrSc. (1993-1997) and doc. MUDr. Miloslav Marel, CSc. (1997-2003). In 1994, the TRN polyclinic facility for Prague 5 became part of the department. In the XNUMXs, interventional bronchology developed, including the treatment of tumor and non-tumor stenoses of the large airways. Furthermore, the care of patients with lung cancer is improving with the introduction of modern radiotherapeutic and chemotherapeutic procedures. In the field of intensive care, non-invasive ventilation support is beginning to be applied. Great attention is also paid to pleural diseases. Following the launch of the lung transplant program in the late XNUMXs, our workplace became a partner of III. surgical clinic of the Motol University Hospital in the preparation of patients for lung transplantation and in the subsequent post-transplant care of these patients.
On December 15.12.2002, 2, the Department of Tuberculosis and Respiratory Diseases (TRN) of the Motol University Hospital became the Department of Pulmonology, Charles University, 2003nd Medical Faculty and the Motol University Hospital. Doc. MUDr. Jaromir Musil, Ph.D. (2010-2000). After the year 2005, there is a further expansion of diagnostic and therapeutic possibilities - new bronchological diagnostic methods, biological treatment of lung cancer, long-acting bronchodilators in the treatment of chronic obstructive pulmonary disease, etc. In 2006 laboratory equipped with a device for examination of exhaled NO in patients with bronchial asthma. The department also participates in clinical studies, grant projects related to early detection of lung cancer and examination of exhaled air condensate are being addressed.
