Introduction of the clinic
The history of the first independent department of the Urological Clinic of the Motol University Hospital began to be written on June 1.6.1984, XNUMX. From the very beginning, the clinic provided medical and preventive care in the entire range of adult urology. Since its inception, it has focused primarily on treatment of oncological diseases, include both surgical and follow - up cancer treatment. Multidisciplinary treatment of oncological diseases takes place in cooperation with the oncology clinic within the Comprehensive Oncology Center (KOC). The clinic also employs several oncology specialists.
The most demanding operations for cancer are performed here in an open way, laparoscopically, as well as the most modern robotic system. Another area of interest is mini-invasive surgery in the form of endoscopic procedures on the upper and lower urinary tract. The urology clinic is also able to provide top care in this area.
The Urology Clinic is one of the few workplaces in the country complete surgical gender reassignment in transsexuals, including subsequent plastic surgery. In cooperation with the IVF center, our andrological outpatient clinic provides sperm collection for assisted reproduction. In the outpatient department, the urology clinic is able to provide comprehensive diagnostics, including highly specialized examinations.
The Urology Clinic is an undergraduate training facility for medics and nurses. It cooperates with ČLK on postgraduate teaching of doctors, it also provides undergraduate teaching of foreign students. The Urology Clinic is an annual co-organizer of the Oncology Symposium, which has become the largest national forum of oncologists. Grant studies are underway at the clinic.

Who is the examination in the metabolic (metabolic-lithic) clinic intended for?
The examination in a metabolic clinic is intended for patients who have a recurrence of urinary stones or who are at risk of recurrence.
How is the examination in the metabolic clinic?
During the first visit to the metabolic clinic, your doctor will discuss with you your previous problems with urinary stones, what diseases you are being treated for and what medications you are taking. Followed by examinations, including ultrasound of the kidneys and bladder. The diagnostics also includes an evaluation of complete blood and urine analyzes from a 24-hour collection, to which you may be invited before your first visit. Based on the results, you will be advised to adjust the menu, or supplemented with drugs to reduce the incidence of other urinary stones.
Is preparation necessary before metabolic examination?
For the first visit, get a report from a general practitioner or urologist. If possible, also bring blood and urine tests from a 24-hour collection.
Instructions for 24-hour urine collection:
Urine collection should take place exactly 24 hours, preferably from morning to morning the next day, so we recommend that you set aside a day off for collection (eg Sunday). Prepare a clean container (eg a plastic bottle) for collection, which you wash with clean, soap-free water.
On the first morning in the morning, you urinate outside the collection container into the toilet (eg at 6:00). After urination, collect all additional urine in a collection container. Store the container in a cool place out of direct sunlight. During collection, drink the usual amount of fluids and eat a normal diet as usual. It is soaked in the collection container for the last time in 24 hours at 6:00. Bring the collected urine to the appropriate laboratory for analysis.

Who is the examination in the urodynamic clinic intended for?
For patients in whom basic urological examinations could not identify the cause of urinary disorders, patients with urinary leaks and patients with neurological diseases affecting urinary tract function.
What is urodynamic examination?
Necessary examination to determine the optimal treatment of functional urinary storage and emptying disorders by measuring pressure, volume and flow conditions in the lower urinary tract.
How is the examination in the urodynamic clinic?
Before the actual examination, there is a detailed interview concerning not only one's own difficulties with urination and retention of urine, but also other diseases of the patient, gastrointestinal function and sexual functions. During the neurourological examination, we evaluate the sensitivity of the skin and the equipment of the reflexes. During the urodynamic examination, the patient urinates into a special device for determining the flow parameters, then the pressures in the bladder, urethra and event are measured using a thin tube. and the rectum when filling the bladder and when urinating. The examination is not painful, but it can be uncomfortable. The scope of examinations and their order are determined by the doctor individually according to the patient's difficulties.
What is the preparation for urodynamic examination?
The patient should receive a maximum of one week old microbiological urine test result from the general practitioner
in case of infection, he should take at least 1 day before the antibiotic examination. The patient should come for the examination
with a full bladder, if the intensity of the difficulty allows.

Who is neurostimulation for?
For patients who suffer from unwanted urine leaks.
The method is especially suitable for patients in whom the current treatment has not led to the relief of problems, for patients who have not tolerated drugs or just do not want to take chemical preparations.
On what principle does the treatment work?
The treatment uses the knowledge that electrical stimulation of the spinal cord for urination through the nerves in the lower limbs can achieve a reduction in bladder activity.
Is this method safe?
Neurostimulation is a safe, painless treatment method. To date, no side effects have been reported with this treatment.
Is this a new treatment method?
Although this method has been widely used abroad for more than 15 years, we are currently the only urological institution in the Czech Republic that performs this treatment.
How does the treatment work?
The treatment takes place on an outpatient basis and can be supplemented with home treatment after the patient's training.

The Prostate Cancer Center was established at the University Hospital in Motol (FNM) in 2010 by prof. MUDr. Marek Babjuk, CSc., Head of the Department of Urology, 2nd Faculty of Medicine, Charles University and FNM in cooperation with MUDr. Jana Prausová, head of the Radiotherapy and Oncology Department of the National Hospital and prof. MUDr. Jiřina Bartůňková, DrSc., Head of the Department of Immunology, 2nd Faculty of Medicine, Charles University and FNM.
We offer patients comprehensive approach in the diagnosis and treatment of prostate cancer:
- surgical removal of the prostate by open or laparoscopic means
- 3D conformal radiotherapy
- IMRT (Intensity-Modulated Radiation Therapy)

Prostate biopsy
Prostate biopsy is indicated when a prostate tumor is suspected. The examination is indicated by the attending urologist or oncologist on the basis of higher values PSA (prostate specific antigen) or suspected palpation of the prostate. During the procedure, min. 10 samples (depending on prostate size and number of previous biopsies). The examination is performed under ultrasound control using a probe inserted into the rectum. To reduce pain during collection, the prostate area is anesthetized with 1% Mesocaine solution. The examination lasts a total of 10-15 minutes, is performed on an outpatient basis, and the patient can leave 30 minutes after the operation.
Instrumentation
We perform prostate biopsy on an ultrasound device using a triplanar rectal probe enabling oblique and perpendicular sampling of prostate tissue.
Preparation
Thorough emptying of the rectum is necessary before the examination itself. In the evening before the examination, 1-2 glycerin suppositories are inserted into the rectum, and 1 glycerin suppository is inserted in the morning on the day of the examination. Suppositories are freely available at the pharmacy. To prevent infectious complications, swallow 2 tablet of the prescribed antibiotic 1 hours before the examination. Swallow another tablet 12 hours after previous use. Antibiotic treatment is prolonged in patients at risk. It is not necessary to be hungry for the examination.
Patients taking antiplatelet agents (eg Anopyrin, Godasal, Trombex, Clopidogrel, Apo-Tic, Plavix, etc.) must discontinue these drugs one week before the examination after consulting their doctor. If Warfarin is used, it is necessary to stop this medicine well in advance and, according to the attending physician, event. convert to low molecular weight heparin. A fresh blood clotting test must be brought for the biopsy.
After biopsy
Because prostate biopsy is an invasive examination, there is a risk of some complications. These are most often bleeding complications - from the rectum, urine or semen, where blood can appear several weeks after the procedure. Others include inflammation of the prostate manifested by fever, difficulty urinating, and pain in the rectum. In this case, it is necessary to see a doctor and treat the condition with antibiotics. Problems with bladder emptying may also occur.
To minimize the risk of developing complications, it is necessary to follow certain rules: on the day of the examination, a rest regime with high fluid intake, to avoid greater physical activity for 2-3 days, to use antibiotics according to the doctor's prescription. If in doubt, consult a doctor.
The biopsy result is available in approximately 2-3 weeks.
We implement at our workplace comprehensive treatment of prostate cancer. This includes in particular surgical removal of the prostate (radical prostatectomy), which we perform either as an open operation or, more recently, as a mini-invasive laparoscopic procedure. For findings with a higher risk, we also perform pelvic lymph node removal. Successful surgery gives patients a fairly high chance of complete tumor removal and complete cure. In this year, we operate on approximately 150 patients with prostate cancer. We also provide hormonal treatment or chemotherapy for selected patients. We then coordinate any radiation treatment in cooperation with the radiotherapy department of the Motol University Hospital
Patients are monitored after surgery in special ambulance, where control examinations are performed regularly according to the protocol - especially regular collection of prostate specific antigen (PSA). We perform this analysis using modern laboratory techniques, the so-called ultrasensitive PSA detection. This allows us to monitor the change in the level of this oncomarker in very low concentrations and thus respond early to any changes in the health status of patients.
Radical prostatectomy carries some risk postoperative complications. It is mainly an erectile dysfunction that occurs in approximately 50% of operated patients. It should be added, however, that adjunctive therapy significantly reduces the percentage of men who do not get an erection. The actual operation involves anatomical and functional changes in the area, which significantly contributes to the ability to retain urine. About the seventh postoperative day, we remove a catheter from the bladder, and most patients go home the same day. However, some may have difficulty fully controlling their urination. These problems are mostly transient and return to full urinary control can sometimes take up to a year. Rarely, there is a case of permanent urine leakage, which can be successfully corrected by aids or implantation of an artificial sphincter.

- All patients who will be operated on at the urology clinic in the following week are invited for an indication visit. Visitation is an important final assessment of the patient's condition.
- The suitability of the proposed surgical solution will be reassessed by the clinic of the clinic's most experienced physicians. The patient's current medical condition will be assessed by an internist and / or anesthesiologist.
- During the visit, a definitive operational program is created as the capacity of the operating theaters allows. The patient will find out when he will be admitted to the hospital, when he will be operated on and other practical information for the hospital.
