Basic information

The Department of Pediatric and Adult Orthopedics and Traumatology, 2nd Faculty of Medicine, Charles University, provides diagnostic and therapeutic orthopedic and traumatological care not only in the catchment area, but also in consulting and consulting activities throughout the country. The clinic has two parts - children and adults.

Main focus and specialization

Within the children's section we specialize mainly in diagnostics and treatment in the following branches of orthopedics

  • our clinic is the leading department of pediatric bone traumatology. The clinic provides care in childhood traumatology with all-day admission of the injured and appropriate emergency treatment.
  • diagnosis and therapy of congenital musculoskeletal disorders
  • conservative and surgical treatment of patients with neurogenic disabilities
  • surgical treatment of bone and soft tissue tumors, bone and joint inflammation of childhood
  • dispensarization and treatment of pediatric hip disease
  • scoliosis therapy.

In addition to standard surgical techniques and treatments, special techniques are also used

  • prolongation of long bones,
  • arthroscopy of children's joints, including reconstructive procedures

Within the adult part is the main focus of the clinic

  • conservative and surgical treatment of osteoarthritis, including revision joint replacements
  • our clinic is one of the leading workplaces with a full range of arthroscopic surgical techniques, including the implementation of reconstructive surgeries of all large joints.
  • as one of the few workplaces in the country, we perform full-scale hip conservation surgery (arthroscopy, open surgeries, pelvic osteotomies).

The clinic also deals with:

  • complex therapy of congenital and acquired foot defects
  • hand surgery
  • solving complex traumatic conditions and consequences of injuries of the musculoskeletal system.

In both parts, ie for adult and pediatric patients, pre-operative rehabilitation treatment by experienced physiotherapists in cooperation with the Rehabilitation Clinic of the Motol University Hospital, prosthetic care and regular school teaching are provided on all weekdays.

Inpatient department

The clinic currently has 105 beds (55 adults, 50 children) in 4 wards and 2 postoperative ICUs.
Standard rooms are 3-bed in the adult part, always two adjoining rooms have a shared shower and toilet. Each adult department has an above-standard single room with a telephone line, TV, refrigerator and private bathroom.
Rooms in the children's section are 2-4 beds and also include rooms with special beds for infants and toddlers with the possibility of single-person accommodation (the capacity of these places is unfortunately limited).
The ICU uses 8 monitored places in the adult part and 4 places in the children's part.

History

The history of the clinic is closely connected with the development of pediatrics in the Czech lands and the establishment of the Children's Hospital in Prague. Post-war establishment II. orthopedic clinic of Charles University in 1945 in the premises of the former Children's Hospital resp. pediatric surgical clinic with an orthopedic specialization belonging to the German University, it starts operating an independent orthopedic clinic with 109 beds, 2 wards for children and 2 for adults under the leadership of prof. Hněvkovského. With the establishment of the Faculty of Pediatrics, Charles University in 1954, the clinic became a teaching base of orthopedics for both students of the Faculty of General Medicine and pediatricians. Gradually, however, she was permanently attached to the Faculty of Pediatrics / now the 2nd Faculty of Medicine, Charles University /. After the relocation of most of the departments and clinics of this faculty to Motol, it remains a detached workplace for a long time, even in less suitable premises. Nevertheless, he was already at the workplace in the 70s, during the management of the clinic prof. Cinderella, joint replacement surgery started, knee replacement with a total endoprosthesis in 1978, even the first in the country. In 1983, a move to Motol finally took place, while the adult wards were temporarily abolished. Prof. Cinderella is replaced by prof. Kubát, progressive surgery is introduced - scoliosis, congenital and systemic defects, limb elongation, pediatric traumatology. The reopening of the part for adults takes place in 1984, again in Karlov nám. No. 1. The clinic subsequently organized the first orthopedic congress in its history in September 1984. Political changes at the end of 1 bring doc. Smetana, which enriches medical activities primarily with the development of surgery for children with DMO.
In 1995, the adult part moved to the temporary bed monoblock in Motol, and until 1998 the adult part of the clinic was waiting to be moved to the so-called "Blue Pavilion" - the Motol bed part for adults. Since the academic year 1999/2000, prof. Tomáš Trč, who was replaced in October 2021 by prof. Vojtech Havlas. At present, the clinic operates in a wide range of fields - orthopedics and traumatology for children and adults. Nationwide superconsulting activities in pediatric orthopedics (congenital and developmental defects, cancers of the musculoskeletal system, neurological conditions, scoliosis, limb elongation) continue, joint replacement surgery, arthroscopic procedures, use of minimally invasive approaches, computer-assisted procedures or genetic therapy ( growth factor therapy).

Undergraduate teaching

Master's degree - medicine
The clinic comprehensively participates in the teaching of orthopedics and bone traumatology within the concept of the 2nd Faculty of Medicine, Charles University. Teaching takes place continuously during six years of full-time study with a predominance of teaching blocks in the 5th and 6th year. In the 4th year, traumatology is taught with an emphasis on injuries of the musculoskeletal system. The teaching of orthopedics ends with a field exam in the 5th year of study. The final state examination in surgery in the 6th year contains questions from acute orthopedics and bone traumatology.

Bachelor study - physiotherapy
In the bachelor's program of physiotherapy at the 2nd Faculty of Medicine, the clinic provides teaching of orthopedics and traumatology in the 2nd year of study in full. The teaching of this subject ends with two exams in Orthopedics, resp. Traumatology.

Bachelor's degree - general nurse
As part of the master's degree in nursing at the 2nd Faculty of Medicine, the clinic participates in the teaching of orthopedics and traumatology in the 1st year of this teaching program.

Master's degree at the Faculty of Education
At the Faculty of Education, Charles University in Prague, the clinic guarantees the teaching of orthopedics and prosthetics at the Department of Special Education.

Within the framework of long-term cooperation with ILF - Department of Pediatrics and Practical Medicine, the clinic works in the teaching of orthopedics, taking into account pediatric practice and the activities of a general practitioner.

Patient story

Pain, limitation of movement and shunt of the limb can significantly affect the patient's life. Take a look at the story of Mrs. Nikola, whose knee replacement and comparison of the deformity of the lower limb significantly improved her life.

Mrs. Nikola came to our outpatient clinic for deformity and severe shunt of a limb. This condition would be caused by a femur infection she suffered as a child. Over the years, the articular cartilage in the knee joint was also destroyed and Ms. Nikola developed significant pain. She went around several medical facilities where she was told that any surgical procedure is extremely risky and not suitable. Mrs. Nikola and I started a long examination process with the aim of reducing possible risks to a minimum. After almost two years, in agreement with the patient, we proceeded to implant a special, so-called suspended knee joint replacement. The implant is unique in its construction, it is connected by a pin between the component of the femur and the tibia and thus enables both lateral and anteroposterior stability. The procedure was performed in our clinic under the guidance of Professor Havlas and there were no complications during the operation. Post-operatively, the patient had to undergo long-term rehabilitation, but already in the early post-operative period it was clear that the procedure had relieved Mrs. Nikole's pain and would lead to an improvement in her quality of life.
