Department of central operating theaters for children

Basic information

The COS Department for Children concentrates all surgical procedures of hospitalized patients in the children's part of the hospital. Apart from the Children's Cardiac Center, no clinic or surgical department has its own operating theaters.

It is our pride that after the reconstruction of the children's part of the Motol University Hospital, we managed to build a workplace that is based on the most modern architectural principles. In addition to state-of-the-art equipment, the operating tract is equipped with electronic security, air conditioning, information technology networks, etc. The movement of material and personnel according to current hygienic-epidemiological rules is strictly observed, ie from clean (sterile) to unclean (used). Three separate basic hygiene zones in the operating rooms are respected, ie clean, protective and sterile.

COS for Children provides services to operational teams in various fields and, according to their characteristics, is organizationally divided into two stations. In addition to routine operations, the doctors of the individual clinics perform a wide range of unique, highly specialized procedures.

Management and staff

The department is headed by a head nurse and a head nurse. The individual stations are led by station nurses, to whom the section nurses are responsible, responsible for the individual sections. The axis of COS for children are specialized nurses in the operating room, who, after three years of experience, supplement their education in the PSS system and pass an attestation exam at the end. For the needs of COS, each instrumentalist specializes in two liaison fields, for which she is designated in day-to-day operations, with one being the main one for her. At the same time, it is educated for universal interdisciplinary use. This meets the necessary time requirements - the possibility of work in emergencies, emergency services and emergencies, while maintaining the requirements for narrow specialization in the field.

The department has several university nurses with bachelor's and master's degrees.

Ambulance

As follows from the mentioned specialization of the department, COS for children do not have an outpatient operation as such. Other facilities in the hospital are used for outpatient operations.

Inpatient department

The COS Department for Children does not have an inpatient department in the true sense of the word. The tract includes waking rooms that allow intensive monitoring of patients in the immediate postoperative period. Reducing the incidence of postoperative complications increases the safety of operated patients and, last but not least, the permeability of operating theaters. The number of nurses in the postoperative wards can then be reduced.

Specialization

At the XNUMXst station for so-called soft surgery are:

in pediatric surgery: game room operations and routine operations in the field of abdominal surgery, incl. microinvasive surgery, liver, bile duct and pancreatic surgery, esophageal thoracic atresia, diaphragmatic hernia, lung resection, chest wall surgery, etc., proctological surgery (anorectal atresia, Hirsprung's disease, Crohn's disease), urological surgery, birth defects and oncosurgery, surgery for burns of children of the lowest age categories and treatment of severe polytraumas in children;

in children's ENT: routine procedures such as adenotomy, tonsillectomy, nasal septal surgery, mastoidectomy, endoscopic procedures and complex procedures of cochlear implants in children, airway stenosis, oncological surgery, injuries (respiratory and digestive tract burns, foreign body extraction);

in ophthalmology: surgery of strabismus, complicated birth defects, congenital glaucoma, retinopathy of prematurity, demanding intraocular operations of the anterior and posterior segment in premature individuals, children and adults, oncological ophthalmic surgery, cataract surgery;

in pediatric gynecology: oncosurgery, reconstructive genital surgery;

in pediatric transplantation: cadaveric organ harvesting, kidney transplantation;

And II. bone surgery stations are:

in pediatric neurosurgeryi: operation of ventriculo-peritoneal short circuits, VP short circuits of newborns of various degrees of complexity and reoperation of older children - VP and VA short circuits; operations for developmental anomalies of the skull and skeleton of the face, meningomyelocele, extensive oncological neurosurgery and surgical solutions of epilepsy;

in pediatric orthopedics: operations of congenital malformations, neurogenically affected children after DMO, dislocation of hip joints, extensive oncological surgery, reconstructive musculoskeletal operations, arthroscopic operations, demanding operations on spinal deformities and musculoskeletal traumatology;

in dentistry: surgical equipment of retinal teeth, facial surgery on the border of neurosurgery - operations of congenital deformities of the facial skeleton and neurocranium, oncological surgery, transplantation of neuromuscular grafts in facial nerve palsy in children and adults and surgical solutions to facial and head trauma;

The scope of the department goes beyond the activities of surgical disciplines. There are also procedures performed by employees of children's internal clinics, oncology clinics, etc. These are bone marrow sampling, airway endoscopy, excision of the liver, kidneys, etc. The department also includes an organ collection center.

During the existence of this department, over 33 operations and roughly the same number of general anesthesia were performed here over 185 years.

Equipment of the device

The department is equipped with state-of-the-art technology. These are mainly highly sophisticated operating microscopes, a system of electric and pneumatic drills, blood separators for returning washed red blood cells from the aspirated blood back into the circulation, devices for ultrasonic crushing and tumor aspiration, electrocoagulation devices for surgical hemorrhage, surgical LASER for cutting and hemorrhage , cryocoagulation devices, working on the principle of extreme tissue freezing, ultrasonic aspirators of lens materials for eye operations, endoscopes, arthroscopes and microinvasive techniques for operation in closed body cavities without the need for large surgical entrances, neuronavigation systems to accurately determine the position of intracranial units and other devices. The equipment also includes excellent anesthesiology technology with perfect monitoring of basic vital functions.

For the professional public

Regular departments are held at the department, and employees indirectly participate in the scientific programs of liaison clinics.

The central operating theaters were originally designed as purely economic units, which were to significantly reduce acquisition and operating costs, especially in larger units. Centralization achieves advantageous reductions in the number of instrumentation serving several industries and significant savings in emergency workers who draw the most money.    

Technologically, the connection to the Central Sterilization Department (CS) is ensured. Instrumentarium, sterile linen, all textile material and disposable aids for planned operations are ordered the day before from CS. Comes clean side elevators or entrances in containers, or other closed packaging. The tools are composed as basic sieves universally applicable, or as special sieves, for certain types of operations. Non-sterile laundry comes from the warehouse or directly from the laundry.

At the end of the operation, the nurse stores the dirty tools in a decontamination container for decontamination and the unused tools back to the original. In this way, elevators or chutes are moved to the dirty part of the central sterilization by the dirty side together with the used laundry and further processed there.

The paramedic only performs qualified work and the nurse can therefore devote herself fully to her expertise and training.

Recently, it has become clear that only facilities organized in this way are able to ensure uniform procedures and standards in maintaining the appropriate hygienic level and regime of operating theaters and to enable a more advantageous system of training of hall instrumentalists.

In this sense, a new field is emerging, covered by several professional companies that bring together employees of central operating theaters and central sterilizations - instrumentalists, anesthesiologists, surgeons, hygienists, epidemiologists, microbiologists and technicians who deal with healthcare issues.

Employees of the department are members of the Society of Hospital Hygiene and the Society of Instrumentation and actively participate in their conferences.

The head of the department is a member of ČSARIM, participates in lecture and publication activities and participates in the teaching of students of the 2nd Faculty of Medicine, Charles University.
