Basic information
The Department of Medical Chemistry and Clinical Biochemistry is a specialized medical, scientific research and educational institution, a joint workplace of Charles University, 2nd Faculty of Medicine and the University Hospital in Motol.
Our workplaces are located on the ground floor, 2nd and 3rd floor of block G of the laboratory complement, between the children's and adult parts of the hospital. Access to the secretariat of the institute and the head, to most laboratories and offices is provided from node 2G - (2nd floor, building G). The reception of ÚLCHKB biological material is also located here.
ÚLCHKB performs a wide range of laboratory tests in the field of clinical biochemistry, pharmacology, toxicology, immunology, endocrinology, etc. for outpatient and inpatient departments of the Motol University Hospital, but also for other departments within the Czech Republic and abroad, continuously 24 hours a day, 7 days a week.

METABOLIC AMBULANCES OF ÚLCHKB
The ÚLCHKB metabolic outpatient clinic provides diagnostics and treatment for patients with disorders lipid metabolismWith disorders of bone metabolism (osteoporosis), with defects nurse .
The metabolic outpatient clinic provides consulting services for the University Hospital in Motol. Blood and urine samples are taken here for laboratory tests and clinical examinations of outpatient patients.
Patients are sent to the ÚLCHKB outpatient clinic by general practitioners or doctors of other specialties. They are ordered with a recommendation either by phone (224) between 9:00 and 12:00 or in person.
The ÚLCHKB metabolic ambulance is located in ground floor of building G. Office hours are from 6:00 to 14:00.
Sampling is performed on working days from 6:00 to 8:00.

The structure of the institute
ÚLCHKB consists of the department of logistics and receipt of biological material, the central biochemical laboratory, the RIA laboratory, the department of clinical pharmacology (TDM and pharmacokinetics), the department of immunochemistry and serology, the department of special methods (HPLC, AAS, FTIR, LC-MS), urine laboratories and laboratories for the analysis of urinary stones, there is also a detached workplace for the determination of chlorides in sweat and a section for POCT. ÚLCHKB provides over 220 different laboratory tests. It examines an average of 2500 biological samples per day, ie 750 examinations / month in a continuous three-shift operation. A number of examinations are also performed for other external medical facilities, eg MTX, cyclosporin A, busulfan, tacrolimus, sirolimus, lead, platinum, IGF-000, determination of antifungals, etc. metabolism, eating disorders in the sense of obesity and malnutrition.
Science and research
Free radical biochemistry, oxidative cell damage and antioxidant systems. Biochemical tissue remodeling and wound healing. Study of connective tissue collagen proteins and their proteinases. Biomarkers of cancer and degenerative diseases. Trace elements, vitamins A, E, K, D. Nutritional hormones. Bone metabolism. Sepsis biomarkers (MDW and MxA protein).
The projects of the Institute of Medical Chemistry and Clinical Biochemistry of the 2nd Medical Faculty of Charles University are focused on research of new biomarkers in malignant tumors (bottoms 2, -2proPSA, mindin, sarcosine), inhibin A as a marker of VVV in prenatal screening, marker of diabetic nephropathy , marker of acute kidney damage (NGAL), monitoring of drug levels (busulfan, ganciclovir), determination of vitamin K1 and K2 levels (MK-4, MK-7) by LC / MS and HPLC, determination of markers of liver fibrosis (hyaluronic acid, collagen, TIMP), determination of selenium and manganese in cerebrospinal fluid using AAS and determination of reference limits in children, research of markers of oxidative stress and lipoperoxidation by fluorimetry and liquid and gas chromatography. The aim of the research is also to determine the reference limits of vitamin K1, MK4 and MK7 and to correlate the values with the values of subcarboxylated osteocalcin and other bone markers.
ÚLCHKB research projects focus on sexual differentiation disorders (steroid 21-hydroxylase, inhibins), nutritional hormones (amylin, orexins), biochemical transplant monitoring (IL-6, cystatin C, alpha-GST), biochemical monitoring of cancer (cathepsin B ), biomarkers of wound healing (metalloproteinases).

It is primarily intended for healthcare professionals, especially doctors and nurses, but the patient can also learn a lot of important information here. The laboratory manual is prepared in accordance with the ČSN EN ISO 9001 standard, taking into account the ČSN EN ISO 15189 standard and the SAK accreditation standards.
Laboratory manual it contains basic information about the organization of the laboratory and the range of services offered, a manual for taking primary samples, information about the pre-analytical phase of the examination, information about issuing results, communication with the laboratory and of course basic information about individual examinations.

Therapeutic drug monitoring
TDM and pharmacokinetic interpretation of drug levels in the context of clinical data
Therapeutic Drug Monitoring (TDM):
It is the determination and subsequent pharmacokinetic interpretation of drug levels in a biological sample (serum, plasma, blood) in the context of clinical data and relevant data. It is a clinical-pharmacological service focused mainly on drugs with a narrow therapeutic index. The goal of TDM is to optimize the therapeutic effect while minimizing the risk of toxicity.
Comprehensive clinical and pharmacological service
Individual adjustment of the dosing regimen in critically ill patients, geriatric patients, neonates, chronic conditions and polypharmacy, where there is a risk of undesirable pharmacokinetic and pharmacodynamic drug interactions.
Prediction and optimization of pharmacotherapy and thus minimizing the risk of toxicity or treatment failure.
Pre-intervention consultation in rational pharmacotherapy for both children and adults.
Consultation in case of unavailability of measurement of drugs with narrow therapeutic index within ATDM (active therapeutic drug monitoring), which consists in computer-assisted simulation of ideal therapeutic limits until the actual concentration is measured (initiation of pharmacotherapy in patients with unstable conditions).
ATDM can be used to reduce doses when co-administering toxic drugs. E.g. ATDM can help with the concomitant administration of aminoglycoside and glycopeptide antibiotics to optimize therapy.
We are currently monitoring the following drugs:
Antibiotics: amikacin, gentamicin, tobramycin, vancomycin
Antiepleptics: ethosuximide, phenobarbital, phenytoin, carbamazepine, carbamazepine epoxide, valproic acid, lamotrigine, levetiracetam, primidone, sulhtiam
Antifungals: itraconazole and hydroxy-itraconazole, posaconazole, voriconazole
Bronchodilation: theophylline
Cytostatics: busulfan, methotrexate, platinum complexes (cis-platinum, carboplatin)
Immunosuppressants: cyclosporin-A, mycophenolate mofetil, sirolimus, tacrolimus

Guide to laboratory findings
The editor of this long-term project is the head of the institute prof. MD Richard Pruša, CSc. and a number of experts from our institute and other workplaces of the Motol University Hospital participate in it as an author. The publication is published by Raabe.

For the publication of the Guide to Laboratory Findings, the Czech Society of Clinical Biochemistry was prof. Průš was awarded the prize "For teaching and education in the field of clinical biochemistry and laboratory medicine". Prof. Průša thanks all colleagues who participated in the creation of the publication.
In recent years, important teaching texts have been published, which are intended not only for students of medical faculties at Charles University, but I can also be a useful study material for students of other universities and also a practical tool for clinical laboratory staff.

Bioanalytical chemistry in examples and exercises is a modern multidisciplinary textbook (exercise book) primarily created for students of the bachelor's degree in Medical Laboratory, which is taught at the 2nd Faculty of Medicine, Charles University, but is also intended for other students of bachelor's and master's courses in medicine, pharmaceuticals, science, medical or chemical-technological faculties that train specialists in the field of biological material analysis, especially specialists for work in clinical laboratories. The textbooks are divided into 25 chapters with a practical appendix with useful relationships and chemical-analytical tables. Selected, didactically related chapters will perform calculations in all areas of practical analytical activities in medical laboratories from basic preparation and preparation of solutions, expression of solutions, preparation of buffers, through titration stoichiometry to calculations in special analytical instrumental methods or processing of analytical results. Much of the more than 530 examples are solved in detail, including an explanation of the logical procedure. This publication is the second updated and expanded edition of the textbook, which is intended for students of Czech medical faculties. A number of leading experts from the 1st Medical Faculty of Charles University, the 2nd Medical Faculty of Charles University, the Medical and Pharmaceutical Faculty of Charles University in Hradec Králové, the Faculty of Chemical Technology of the University of Pardubice and workplaces from Olomouc, Ostrava and Třinec participated in the creation of these scripts. The Institute of Medical Chemistry and Clinical Biochemistry of the 2nd Medical Faculty of Charles University is a part of the accredited medical facility of the University Hospital in Motol. The medical facility was successfully investigated Joint Accreditation Commission of the Czech Republic and met the criteria for management and continuous quality improvement according to national accreditation standards.
